The Story of the Van

In 2010, the CCMF Board of Directors recognized that the agency’s original van (donated to CCMF by St. Joseph’s Hospital) was going to need to be replaced at some point in the not-too-distant future. CCMF began an aggressive fundraising campaign but by 2014 when the original van was no longer safe to drive, were still significantly short of the $60,000+ price tag of a new van.

We are eternally grateful that The Judith & Norman Alix Foundation heard of our plight and donated the entire $60,000 to cover the purchase of a van and to have it fully equipped to safely transport our medically fragile. The van is available to those who require transportation of their medically fragile loved one(s). We do not charge for the use of the van but do ask that you replace the gas that you use for your trip.

We can also provide training to those wishing to become qualified to drive our van.

Qualify to Drive The Van

CRITERIA TO QUALIFY TO DRIVE THE VAN

  1. Must be, or willing to become, a CCMF member at large. ($20 per year)
  2. Must possess a current, valid Class G driver’s license and provide a copy of this license to CCMF. Understand that a Driver’s abstract will be completed by CCMF’s insurance provider.
  3. Must provide the name of your Insurance provider and the Policy No.
  4. Complete Q-Straint on-line training.
  5. Complete an orientation of the use of the van by a CCMF approved person. Procedures that are required of drivers, are reviewed during this orientation.
  6. Upon provision/agreement of the above criteria, the Chair of our Transportation Committee will review the request and provide a decision.

The Judith & Norman Alix Foundation heard of our plight and donated $60,000 to cover the purchase of a new van and have it fully equipped to accommodate the wheelchairs safely in addition to having it loaded with the necessary medical equipment to safely transport our medically fragile. Heartfelt thanks to this Foundation for coming to our rescue. We are forever indebted!

We transport our medically fragile to where they need to go. Membership is a requirement to utilize the van service. Please contact the office for an application form.
